ZOLL^® HEROES FOR LIFE TERMS AND CONDITIONS

I agree that by submitting my contact information below to ZOLL, representatives of ZOLL may contact me to discuss my sudden cardiac arrest (SCA) rescue experience with a ZOLL automated external defibrillator (AED). I grant permission to ZOLL and the public safety organization involved with my rescue (if applicable) to use information relating to my SCA rescue experience involving a ZOLL AED. This includes, but may not be limited to, my name, my photo, general medical condition, location of incident and information regarding the rescue.

This information may be shared with the general public through media outlets, ZOLL partners and distributors, and ZOLL’s external communications (including, but not limited to, ZOLL’s websites, social media, and email communications).

ZOLL may also use this information for the following:

• To create a recognition award as a sudden cardiac arrest survivor or for the rescuer(s)’ lifesaving work.
• For educational outreach, including, but not limited to, case studies, newsletters, and news stories.
• To allow members of the media to contact and potentially interview me about my experiences with a ZOLL AEDs. I understand my interview may be edited and used in whole or in part, and may be produced, duplicated, and distributed by participating media organizations within and outside the U.S.
